Volunteer Waiver

Mission: Take the Church outside the walls, bringing light into the darkness with the life-changing messages of the Gospel of Jesus Christ.
Vision: Giving everyone a chance to say yes to Jesus through focused-driven evangelism.

Hills Alive Volunteer Disclaimer and Release of Liability

By signing this document, I agree to the following statements:

  • I certify that I have read this form and agree to abide by all the guidelines.
  • I am volunteering for LifeLight Hills Alive—a ministry of LifeLight Communications. (herein after referred to as Hills Alive)
  • I, the Volunteer, release and hold harmless Hills Alive and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work with Hills Alive.
  • I understand that this Waiver discharges Hills Alive from any liability or claim that I, the Volunteer, may have against Hills Alive with respect to bodily injury, personal injury, illness, death, or property damage that may result from my participation on Hills Alive’s work site.
  • I also fully understand that Hills Alive does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage. I hereby release Hills Alive from any claim whatsoever which arises or may arise in the future due to any first aid treatment or other medical services that are conducted in connection with an emergency during my time with Hills Alive.
  • I understand that my time with Hills Alive may include various activities that may be hazardous to me, and I hereby expressly and specifically assume the risk of injury or harm in these activities and release Hills Alive from all liability for injury, illness, death, or property damage resulting from the activities of my time with Hills Alive.
  • I grant unto Hills Alive all rights, title, and interest in any and all photographic images and video or audio recordings that are made by Hills Alive during my work with Hills Alive, including, but not limited to, any royalties, proceeds, or other benefits that are derived from such photographs or recordings.
  • I expressly agree that this Waiver is intended to be as broad and inclusive as permitted by the laws of the State of South Dakota and that this Waiver shall be governed by and interpreted in accordance with the laws of the State of South Dakota.
  • I agree that in the event that any clause or provision of this Waiver shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.
  • I understand that Hills Alive will conduct a pre-volunteer background investigation and hereby grant permission to Hills Alive to do so.
    • The information from this check will be used to assess my suitability for volunteering.
    • I understand and acknowledge that I will not be allowed to volunteer if I am listed on the National Sex Offender Registry for the safety and protection of participants.
  • If the volunteer has indicated they are under the age of 18, a parent or legal guardian consent signature & information are required on behalf of the minor.
  • I understand that Hills Alive expects volunteers to reflect the love of Christ in our actions, lifestyle and communication with others.
